Waiver, Assumption of Risk, and Release of Liability

Goldenstars Boxing is a combat sports facility. Boxing involves intentional striking and physical contact between participants. Boxing and combat sports carry a higher risk of serious injury than typical fitness or recreational activities. By choosing to participate, I am voluntarily engaging in an activity that may cause significant physical harm.

I understand that boxing and related training involve inherent risks, including but not limited to:

• Strenuous physical exertion
• Slips, falls, collisions, and equipment-related injuries
• Sprains, strains, fractures, cuts, bruises
• Concussions, head injury, and other serious injury
• Injury from contact drills and sparring, including for non-competitors
• Illness or other health complications
• Aggravation of pre-existing conditions

These risks can be serious and may result in permanent injury or death. I voluntarily choose to participate, knowing these risks exist.

If I participate in sparring or contact drills, I understand risk increases substantially.

I agree:

• Sparring is voluntary and may be limited or prohibited by coaches at any time
• I will follow all sparring rules and coach instructions
• I will use required protective equipment as directed
• I will stop immediately if instructed or if I feel unwell, dizzy, injured, or unsafe

Goldenstars Boxing may refuse or stop sparring participation at any time for safety, skill level, conduct, or any other reason.

A separate sparring acknowledgment is required in the form below before signing.

I represent that:

• I am physically and medically able to participate
• I have no condition that would make participation unsafe, or I have consulted a medical professional and been cleared to participate
• I will promptly inform coaches of relevant medical conditions or injuries that affect safety
• I will not participate under the influence of alcohol, cannabis, illegal drugs, or impairing medication

I have disclosed any known pre-existing medical conditions in the form below (or written "None"). I accept full responsibility for any aggravation of those conditions.

Goldenstars Boxing does not provide medical advice. I am responsible for consulting a physician before beginning training and for monitoring my own health throughout participation.

I agree to follow gym rules, safety guidance, and coach instructions. I understand that failure to follow rules increases risk of injury to myself and others.

I accept full responsibility for my actions and behavior while participating.

To the fullest extent permitted by law, I release and waive all claims against Goldenstars Boxing and its owners, coaches, staff, contractors, volunteers, agents, and representatives ("Released Parties") for any injury, loss, damage, or expense arising out of or related to my participation in Activities, including sparring, whether caused by negligence, breach of statutory duty, or otherwise.

This release includes claims arising from:

• Training instruction or supervision
• Facility conditions
• Use of equipment
• Contact with other participants
• Acts or omissions of the Released Parties, including negligence

This release does not extend to gross negligence, wilful misconduct, or any liability that cannot be excluded under applicable Alberta or Canadian law.

I agree to indemnify and hold harmless the Released Parties from third-party claims, demands, damages, or expenses (including reasonable legal fees) arising from:

• My own negligent or wrongful acts or omissions during Activities
• My breach of this waiver, gym rules, or coach instructions
• My violation of any applicable law during Activities

This indemnity does not require me to pay the Released Parties' costs in any claim I bring against the Released Parties.

I understand that I am responsible for my own property. I agree Goldenstars Boxing is not responsible for loss, theft, or damage to personal items.

If I require emergency medical assistance, I authorize Goldenstars Boxing to seek reasonable assistance on my behalf. I understand I am responsible for any related costs.

I will provide an emergency contact below who can be reached in case of injury or medical emergency during Activities.

By signing this waiver, I grant Goldenstars Boxing permission to photograph or record me during Activities and to use my likeness for promotional purposes (website, social media, advertising) without compensation.

If I do not want my photo or video used for promotional purposes, I will email contact@goldenstarsboxing.com - either before signing this waiver or at any time afterward - to be added to the no-photo list. My request will be honoured for future use.

Withdrawal of consent for future use does not require Goldenstars Boxing to recall material already published.

Personal information collected through this waiver is handled according to the Goldenstars Boxing Privacy Policy. By signing, I confirm I have access to that policy.

This waiver is effective from the date I sign it and applies to all my participation in Activities until either (a) I revoke it in writing to contact@goldenstarsboxing.com, or (b) Goldenstars Boxing publishes a new version of this waiver, in which case I may be asked to sign the updated version before continuing.

If any part of this waiver is found unenforceable, the remaining parts will continue in full force.

This waiver is governed by the laws of Alberta and applicable laws of Canada. I agree that disputes will be resolved in Alberta.

I certify that:

• I have read and understood this document in full
• I have had sufficient opportunity to ask questions before signing
• I am signing freely, voluntarily, and not under any duress or pressure
• I am at least 18 years old, OR a parent/guardian is co-signing on the section below
• I understand that by signing I am giving up important legal rights, including the right to sue for negligence

Part A - Consent to Participation

I am the parent or legal guardian of the minor named below. I consent to the minor's participation in Activities at Goldenstars Boxing. I have explained the risks described in this waiver to the minor in age-appropriate terms, and the minor has agreed to follow all gym rules and coach instructions.

Part B - Waiver of My Own Claims

I personally waive and release any claim I may have against the Released Parties arising from my child's participation, including but not limited to claims for medical expenses I may pay on the minor's behalf, loss of services or guidance, or emotional distress.

I acknowledge that under Canadian law I cannot waive the minor's own legal rights to bring a claim, and this section is not intended to do so.
